Job Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and managing the care of patients in a hospital or home health setting. Your role will involve assessing patient needs, developing care plans, and ensuring that patients receive appropriate medical services and resources.
Responsibilities:
- Conduct comprehensive patient assessments to determine their medical, social, and psychological needs
- Develop individualized care plans based on patient assessments and in accordance with Medicare guidelines
- Collaborate with healthcare professionals to coordinate patient care and ensure the delivery of quality services
- Monitor patient progress and make necessary adjustments to care plans as needed
- Provide education to patients and their families regarding their conditions, treatment options, and available resources
- Review medical records and documentation to ensure accuracy and compliance with HIPAA regulations
- Advocate for patients' rights and ensure that they receive appropriate care based on established standards
- Collaborate with insurance providers to obtain authorizations for necessary medical services
- Facilitate discharge planning and coordinate post-discharge care for patients transitioning from the hospital or home health setting
Qualifications:
- Bachelor's degree in nursing or a related field (preferred)
- Previous experience as a Case Manager in a hospital or home health setting
- Strong knowledge of anatomy, medical terminology, Medicare guidelines, and HIPAA regulations
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Ability to assess patient needs, develop care plans, and make informed decisions regarding patient care
- Proficient in reviewing medical records and documentation for accuracy and compliance
- Familiarity with ICU settings is a plus
